We're hiring for a full-time Home Health Physical Therapist (PT) for the Volusia County location.

Physical Therapist (PT)
to join our team in the Volusia
County area.

The Physical Therapist will provide skilled therapy intervention and treatment to customers in the home setting. This care includes appropriate utilization of treatment plans and assessment skills. Careful monitoring of the customer s status and progress toward goals as well as compliance with physician s orders. Responsible for educating customers and caregivers while preparing the customer/caregiver for discharge as appropriate.

Education and Experience Requirements:

  • Active Florida PT license
  • A minimum of 1 year of PT experience in home health
  • A valid CPR certification and driver s license
  • Work positively and favorably with customers, families, and staff.
  • Ability to relate positively and favorably with clients and staff.
  • Demonstrate good oral and written communication along with good documentation skills.
  • Proficient with computer software/hardware.
  • Bachelor of Science in Physical Therapy from an accredited program
  • Successful completion of National Certification Examination for Registered Physical Therapist.

primary responsibilities include:

  • Provide skilled physical therapy in accordance with the physician s plan of treatment.
  • Assist the physician in the evaluation and assessment of the client s level of functioning by applying appropriate tests and assist in the development and revision of the plan of care.
  • Treat clients to relieve pain, develop and restore function and maintain maximum performance using appropriate physical therapy treatments, modalities and procedures as stated in the physical therapy plan of care.
  • Observe, record, and report the client s reaction to treatment and any changes in the client s condition to the physician, Director of Nurses and Case Manager.
  • Educate client/caregiver and health care team in the phases of physical therapy in which they may assist the client, in the use of prosthetic and orthotic devices, and durable medical equipment, as appropriate.
  • Assess and regularly reevaluate the needs of the client.
  • Develops implements and revises the client s plan of care.
